After filling out the four sections of this form, you will be able to manage your child or children's registration at Music Centre electronically.

Your data will be used by staff at Music Centre to enable the running of the centre, and for no other purpose.

Your connection to this site is encrypted for your security.

Your chosen password will be stored in an encrypted format, so it cannot be read by anyone.

The data you provide about yourself and your child/children may be retained by the Music Centre for up to 5 years in accordance with the requirements for Gift Aid.

If you are a UK taxpayer, the value of your payment to Derby Music Centre can be increased by 25% under the Gift Aid scheme at no extra cost to you. If you consent to Gift Aid, Derby Music Centre may ask you to complete a declaration for our records. You can change your mind at any time by letting us know.
